    Return to Campus Selection Form

In preparation for an eventual return to campus, LAUSD is asking all families to complete the survey regarding your choice of instructional models for your child. Please submit forms by December 6, 2020. Please submit a separate form for each of your children. On the survey, your choice of program model (online or hybrid) will become available after you enter your child's grade level. Your student ID can be found on your child's report card or through Parent Portal. If no response for your child is submitted, your child will be signed up for the hybrid model.
